OFFICE HOURS

For information or to schedule an appointment, please call us at 847-679-6707.

Monday – Friday 8:15 a.m. – 4:00 p.m.
Saturday 8:30 a.m. – 12 p.m.
We are closed everyday between 12:00 p.m. and 1:00 p.m. for lunch.

We can accommodate most same day visits for our patients.

If you need to reach someone after hours, there is always a provider on call.

CANCELLATION POLICY

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.

INSURANCE

We also participate with many insurances’ plans. Medicare is accepted, Medicaid is not. If you are within an HMO your primary care physician has to be selected and you will need to talk to your insurance company to do this. If you are looking to change within insurances, you need to call the insurance company you’re looking into to confirm your doctor is in network.
